It was visually inspected and shows scuffs and smudges with slight warping around the edge but doesn't seem to interfere with the playing grooves. No chips or cracks. The inlay is worn in the center but otherwise in very good shape. No original sleeve, but will be shipped in an acid-free sleeve and packed securely between cardboard.
