Description: This is an incredibly rare application form from 2002 for the Pokemon Card Fan Club Exclusive to Japan. This Application form was used to try and obtain many rare Japanese Exclusive Pokemon Cards, including the amazing Pokemon Fan Club Porygon and the highly sought after, Pokemon Fan Club, Shining Magikarp. I have never seen another one of these forms, especially in this condition – available for sale. Even finding just an image online, is very difficult. I’m unsure if it’s gradable, but if it is, I assume it would receive a very high grade as it’s incredibly clean (please refer to the images for condition, or feel free to send an inbox for more info) Even if it isn’t graded, it would make an absolutely amazing piece of Pokémon history for any serious collector, framed and displayed! All in all, this is an incredible piece of Pokémon history that is incredibly rare, absolutely stunning and a must have!
