The felt has been removed from the bottom but it is included, I didn't want to reglue it myself because all I have here is superglue, and I doubt if that would be the best choice. No chips or other damage to the glass, there is light wear to the brass. 2 prong, polarized plug with green cord. Push in switch on socket itself. Stamped Leviton and Heyco. Light bulb not included, it's just one more thing to worry about breaking and is just a standard, common bulb.
